An enrollment verification is an official document which provides enrollment status. It does NOT include individual course or grade information and does not provide GPA. An enrollment verification document can be used whenever enrollment verification of student status is required for scholarship purposes, medical coverage, loan deferments, jury duty, car insurance, etc.
Requesting Verification of Enrollment
To request an enrollment verification letter:
- Click on the button below to download the Enrollment Verification Form.
- Complete the form. Be sure to indicate at the top right what information needs to be verified.
- You may mail in this form to the Registrar’s Office; or
- You may also come in person to any campus to obtain an enrollment letter.
- Official verification of enrollment is available after the published drop date for the semester. Prior to the drop date, CSM issues a pre-registration letter.
