How to Request Accommodations

To request reasonable accommodations and support services through the Office of Disability Support Services (DSS), please complete these five simple steps:

1. Complete application.

The first step is to complete the Application Form every semester.

Accommodations may change from semester to semester and will not be implemented retroactively.

If you need assistance completing the form, please contact your coordinator.

All accommodations are determined on a case-by-case basis, according to the needs of the individual and the individual’s specific program of study for a particular semester.

2. Submit appropriate documentation confirming your disability.

Documentation should be current and will be evaluated on a case-by-case basis. Documentation may be emailed, mailed, or dropped off in person to your coordinator. 

3. Meet with us.

After receiving your paperwork, the DSS staff will schedule an initial appointment to assist you with access, accommodations, and services that you may need.

4. Connect with your instructors.

After you receive the email notice confirming your approved accommodation plan. Present your instructors with your Approved Accommodations Plan to coordinate accommodations. It's good practice to also have a confidential discussion with your instructor about your accommodations shortly after giving them the letter. You aren't required to disclose medical information to your instructors.

5. Reflect on your accommodations and revisit with us.

Pay attention to what's working, what's not working, and what kind of barriers you're still experiencing throughout the semester. Your accommodations aren't set in stone.

Please notify your coordinator right away if you encounter any serious academic barriers at CSM.

It is very important to have your documentation correct and up to date to receive services. Temporary accommodation can be provided while you are obtaining your documentation. A list of psychologists in the metro area can be provided by DSS for those who need psychoeducational assessment.
