MLT Program Eligibility Assistant

Thank you for your interest in the Medical Laboratory Technology (MLT) Program at CSM. 

Please complete the following questionnaire as it provides the necessary next steps in ensuring your eligibility for entrance into the program.

• Yearly program cohorts begin each fall semester; therefore, summer courses may be necessary prior to admission.
• The program utilizes a high level of technology. Students should be able to access their CSM email, Microsoft 365; including Teams, and myLearning accounts without assistance.  Students can receive help with these accounts from the CSM IT Help Desk. A smart phone or tablet with picture taking and uploading capabilities are necessary for lab assignments.
• MLT students are required to purchase grape-colored unisex scrub uniforms, yearly web-based subscriptions for course assignments, white dress lab coat (at the end of the program), and background checks/drug screen/health records in addition to course costs and fees.
• Please schedule an appointment with an Academic Advisor as soon as possible to create your academic plan and start course registration.
• All documents submitted in this questionnaire must also be submitted officially to the Registrar using the Official Transcript Evaluation form. Entrance into the program will be delayed if transcripts are not provided.
